Information needed when filling out a PCard Support Form:
When - When will you be using the product?
Where - Where will you be using the product?
Why - Why do you need it?
Who - Who will be using it?
What - What is the product?
If you have been issued a refund, a PCard Support form will need to be filled out for the original purchase and the refund. Please give as many details as possible. Why was the refund issued and what was the original purchase?
Per policy, you need to turn in your completed PCard Support form with the signed receipt within 21 days of the purchase. Complete budget needs to be indicated (budget, fund, and cost center) and all signatures need to be acquired.
Conference Registration forms must include full name of the conference (no initials), dates of the conference, where it is being held, and the purpose of attending (give a talk, present a poster, research collaboration, etc.)
There is a separate form for memberships as additional approval signatures are needed.
Memberships cannot be charged to grants, only general funds, unless the membership and registration are cheaper than the non-member registration. As this is saving the sponsor money it can be placed on research. Proof of this needs to be provided with the form.
Transactions are not to be split into two separate transactions to bypass your PCard limit. If a purchase is going to be over your limit, you can submit a Purchase Order or request your PCard limit to be temporarily raised.
